Privacy Notice & Consent Form
I am committed to complying with the terms of the General Data Protection Regulation (GDPR) 2018 and to the responsible and secure use of your personal data. As a therapist I have a legitimate interest in processing your personal data in order to provide you with counselling services. The purpose of this statement is to let you know what personal information I will collect and why, how long I keep it for and your rights in relation to your personal data.
Information about you
I collect personal information from you in order to set up an initial appointment and to make contact with you as necessary from that point on. This information includes your full name, contact details, your availability and other relevant personal information. When you attend an initial assessment session I collect additional information that will help me tailor my therapy to your needs including about your background, medical history, and emergency contact details. During ongoing therapy, I will take brief notes after each session in order to monitor my work and these are always non-identifiable and are kept in a locked, secure location. Once you finish counselling, all data regarding your therapy is stored securely for 7 years and then destroyed
My use of this information
Your personal information will only be used to provide you with my services or to give you information relating to my services. I will not share your personal details with any other person or organisation without your knowledge and permission, unless there is a legal requirement to do so, for example if there is a child or adult safeguarding issue, or a perceived risk of harm. A breach of confidentiality occurs when a person shares information with another in circumstances where it is reasonable to expect that the information would be kept confidential
I will take all reasonable precautions to prevent the loss, misuse or alteration of the information you give me. Any information stored electronically will be password protected. Any paper-based information will be kept in a locked secure cabinet.
Your rights over your personal data
If you would like to see the information I hold about you, or would like to correct, update or delete any records, please contact me on email@example.com. If you have any concerns about my use of your data, please let me know and I will do my utmost to resolve any concerns you have. If your concerns are not resolved to your satisfaction, you can contact the ICO. You may withdraw your consent for me to hold your data at any time and it will be securely destroyed. However, if you do this while actively receiving therapy your therapy would need to come to an end.
